Job Details

• Weekly Estimated Pay: $2,050-$2,160

• Travel Radius: Up to 75 miles from the assigned care center
• Patient Load: Approximately 5-8 patients per day
• Productivity Expectation: 30 points per week
• Call: Potential rotating on-call coverage
• Schedule Notes: No guaranteed hours; overtime must be pre-approved
• Charting System: HomeCare HomeBase (HCHB) and PointCare; OASIS proficiency required
• Other Details: Local candidates accepted; mileage reimbursement available

Job Requirements

• Current Registered Nurse (RN) license in hand
• Minimum 1 year of adult home health experience or 1 year of hospice experience, depending on specialty
• Recent experience with HomeCare HomeBase required; OASIS proficiency required for RNs
• Current CPR/BLS certification
• Reliable transportation, valid driver’s license, and auto insurance
• Comfortable traveling daily to patient homes and attending periodic office meetings
• Willingness to cover a broad territory and manage a dynamic home care schedule
• Ability to complete documentation within 24 hours of each visit

Responsibilities

• Provide skilled nursing care in patients’ homes according to physician orders and agency policies
• Use the nursing process to assess, plan, implement, and evaluate patient care
• Coordinate and direct care delivery to support safe, high-quality outcomes
• Supervise and evaluate Home Health Aides and Licensed Practical/Vocational Nurses as needed
• Complete Start of Care, wound care, follow-up visits, and other home health services
• Travel between patient visits while maintaining timely charting and visit documentation
• Work collaboratively with the care team to support patient needs and continuity of care
• Maintain professionalism, flexibility, and strong communication throughout the assignment
