• Charting: Epic and OASIS required
• Patient Population: Adults and geriatrics

Job Requirements

• Active Oregon RN license required at submission
• Minimum 1 year of skilled home health nursing experience required
• BLS certification required
• OASIS experience required
• Experience documenting in Epic required
• Ability to provide independent nursing care in the home setting
• Reliable transportation required for community visits
• Ability to work 3 of the 5 major holidays during the contract
• Comfortable with a 50-mile local radius and occasional travel between nearby patient locations

Responsibilities

• Provide skilled nursing care to adult and geriatric patients in their homes
• Complete patient assessments, care plans, and routine follow-up visits
• Deliver wound care, wound vac management, and disease management support
• Manage PICC lines, G-tubes, ostomy care, and catheter care education
• Document accurately in Epic and complete OASIS assessments as required
• Coordinate care with interdisciplinary support services as needed
• Support patient safety, education, and continuity of care in the community
• Maintain productivity expectations while delivering compassionate, high-quality care
