Registered Nurse Job Requirements

• Active Registered Nurse (RN) license in Massachusetts
• Minimum 1 year of nursing experience required; candidates with 6 months of nursing experience and a BSN may also be considered
• CPR certification required
• Successful completion of pre-hire medication testing
• Home health nursing and IV experience preferred
• BSN and strong computer skills preferred
• Ability to provide safe, professional care in a home care and home health setting

Home Health RN Responsibilities

• Complete comprehensive nursing assessments upon patient admission
• Evaluate physiological, psychosocial, environmental, and health history factors to support quality patient care
• Develop, implement, and revise patient plans of care as needed
• Use HHRG values to help establish visit frequency and interdisciplinary utilization
• Participate in interdisciplinary case conferences and evaluate plan effectiveness
• Provide and document supervision of the Home Health Aide plan of care every 14 days
• Notify physicians and other care providers of significant changes in patient condition
• Complete admission, discharge, and other required documentation in a timely manner
• Maintain clear, concise narrative notes that reflect skilled nursing care, teaching, and patient/caregiver response
• Obtain and clarify valid physician orders for all care delivered
• Coordinate safe and appropriate discharges and document all related paperwork
• Support community resource referrals and assist patients in achieving positive outcomes
