Registered Nurse Job Requirements

• Active Registered Nurse (RN) license in Massachusetts
• Minimum of 1 year of nursing experience required; candidates with 6 months of experience and a BSN may also be considered
• Current CPR certification required
• Successful completion of pre-hire medication testing
• Home care experience preferred
• IV experience preferred
• Strong computer and documentation skills preferred
• BSN preferred

Home Health Nursing Responsibilities

• Perform comprehensive nursing assessments on admission, including physiological, psychosocial, environmental, and health history factors
• Develop, implement, and revise individualized plans of care based on patient needs and assessment findings
• Use HHRG values appropriately to guide visit frequency and interdisciplinary utilization
• Monitor patient progress and ensure annual outcomes align with national benchmarks
• Refer patients to community resources as appropriate
• Participate in interdisciplinary case conferences and evaluate the effectiveness of the plan of care
• Provide and document supervision of the Home Health Aide plan of care every 14 days
• Notify physicians and other care providers of significant changes in patient condition
• Complete admission, narrative, and discharge documentation accurately and on time
• Obtain and clarify valid physician orders for all care delivered
• Coordinate discharges and ensure patients, providers, and records are updated appropriately
• Perform other duties as assigned to support safe, high-quality patient care
