Home Health RN Job Details

• Weekly Estimated Pay: $2,120–$2,230
• Healthcare Setting: Home health
• Assignment Location: Leominster and surrounding Massachusetts communities
• Start Date: October 2, 2026
• Assignment End Date: December 31, 2026
• Contract Duration: 13-week nursing contract
• Schedule: 8:00 a.m. to 4:30 p.m., daytime shifts
• Hours: 40 hours per week, with a 32-hour weekly guarantee
• Weekend/On-Call Requirement: No regularly scheduled weekend or on-call requirement
• Additional Details: Mileage reimbursement between patient visits; casual professional attire or scrubs permitted

Home Health Registered Nurse Requirements

• Current Registered Nurse (RN) license valid in Massachusetts
• At least 1 year of professional nursing experience; home health or visiting nurse experience preferred
• Previous OASIS assessment and clinical documentation experience
• Bachelor’s degree in nursing or a related field
• Strong clinical judgment, organization, and time-management skills
• Excellent interpersonal and patient communication skills
• Ability to travel throughout the assigned service area and provide skilled nursing care in patients’ homes

Home Health RN Responsibilities

• Perform comprehensive nursing assessments and develop individualized care plans for home health patients
• Complete accurate OASIS assessments and maintain timely, thorough clinical documentation
• Provide skilled nursing interventions, including medication management, wound care, disease education, and symptom monitoring
• Monitor patient progress and update care plans in collaboration with the interdisciplinary care team and prescribing providers
• Educate patients and caregivers about treatment plans, medications, disease management, and safety precautions
• Recognize changes in patient condition and promptly communicate concerns to the appropriate members of the healthcare team
• Coordinate care with physicians, therapists, social workers, and other healthcare professionals
• Support quality outcomes while meeting established productivity and home health visit expectations
• Build trusting relationships with patients and families through compassionate, respectful care
