Join a collaborative outpatient care coordination team as a Patient Care Coordinator RN in Portland, Oregon. This on-site travel nursing opportunity is ideal for an experienced RN with case management skills, EPIC experience, and an active Oregon, Washington, or compact RN license. You will help coordinate care for patients across the healthcare continuum while supporting an organized, patient-focused clinical environment. - Location: Portland, Oregon 97232
- Healthcare Setting: On-site outpatient care coordination
- Weekly Estimated Pay: $2,750–$2,890
- Start Date: July 5, 2026
- End Date: October 3, 2026
- Contract Duration: 13-week travel nursing contract
- Schedule: 8-hour shifts; schedule provided as 12:00 a.m.–8:30 a.m.
- Hours: 40 hours per week
- Weekend and Holiday Coverage: Required, including rotating coverage across the week
- Additional Requirement: Willingness to float to at least one neighboring regional location
RN Job Requirements
- Active Oregon RN license, Washington RN license, or compact RN license maintained on file
- At least 1 year of experience in case management, patient care coordination, nurse care management, or a related role
- Current Basic Life Support (BLS) certification
- Experience with EPIC electronic medical records (EMR)
- Working knowledge of Medicaid and Medicare processes
- Strong communication, organization, clinical documentation, and care coordination skills
- Ability to work weekends and holidays as scheduled
- Flexibility to support neighboring regional locations when needed
Patient Care Coordinator RN Responsibilities
- Coordinate patient care plans and healthcare services across multidisciplinary teams
- Assess patient needs and support appropriate transitions of care
- Communicate with patients, families, providers, and community resources
- Document assessments, care coordination activities, and follow-up plans in EPIC
- Help patients navigate Medicaid, Medicare, and other healthcare resources
- Identify barriers to care and collaborate on practical solutions
- Monitor follow-up needs, referrals, and ongoing care coordination activities
- Provide coverage in neighboring locations as assigned
