LTAC Registered Nurse Job Requirements
- Current Registered Nurse (RN) license eligible for practice in Pennsylvania
- Minimum of 2 years of bedside clinical nursing experience
- Current BLS certification required before the first shift
- Current ACLS certification required before the first shift
- Strong patient assessment, communication, organization, and clinical judgment skills
- Ability to work night shifts and collaborate effectively with an interdisciplinary healthcare team
- Commitment to safe, patient-centered care in a long-term acute care setting
LTAC Nursing Responsibilities
- Complete comprehensive nursing assessments for patients upon admission and throughout the stay.
- Develop, implement, evaluate, and update individualized plans of care using the nursing process.
- Provide direct patient care while supporting medical stabilization, rehabilitation, education, and functional goals.
- Monitor patient conditions and promptly communicate significant changes to physicians, charge nurses, and other members of the care team.
- Document assessments, interventions, patient responses, and care coordination accurately and in a timely manner.
- Participate in interdisciplinary team conferences and contribute meaningful updates regarding patient progress and ongoing needs.
- Initiate discharge planning and provide patients and families with education to support a safe transition of care.
- Delegate appropriately and provide direction, training, and supervision to LPNs, CNAs, and nursing support staff.
- Promote patient and staff safety by following infection prevention, safe patient handling, workplace safety, and facility procedures.
- Prioritize assignments effectively, maintain a calm and encouraging presence, and contribute to a positive unit environment.
- Assume charge responsibilities after orientation as requested and pursue ongoing professional development.
