We are seeking a compassionate and experienced Home Health Registered Nurse (RN) to provide skilled nursing care throughout Indianapolis, Indiana. This 13-week home health nursing contract offers a consistent Monday–Friday schedule, guaranteed full-time hours, and the opportunity to support patients and families through personalized, in-home care. Located in Northwest Indianapolis, the area offers a welcoming community, convenient amenities, beautiful parks, and easy access to the cultural and recreational attractions of central Indiana.
Home Health RN Job Details
- Weekly Estimated Pay: $1,900–$2,000
- Location: Indianapolis, Indiana
- Healthcare Setting: Home health
- Schedule: Monday–Friday, day shift
- Shift Duration: 8-hour shifts
- Hours: Guaranteed 40 hours per week
- Contract Duration: 13-week contract
- Start Date: October 12, 2026
- End Date: January 10, 2027
Home Health Registered Nurse Requirements
- Active Registered Nurse (RN) license eligible for practice in Indiana
- At least 12 months of home health nursing experience within the past 3 years
- Previous OASIS experience required
- Experience with HCHB required
- Ability to complete comprehensive patient assessments and accurate clinical documentation
- Ability to meet a minimum productivity expectation of 30 points per week
- Strong communication, organization, and independent decision-making skills
- Reliable transportation and ability to travel throughout an assigned home health service area
Home Health Nursing Responsibilities
- Provide skilled nursing care to patients in their homes throughout the Indianapolis area
- Perform comprehensive patient assessments and develop individualized plans of care
- Complete OASIS assessments accurately and within required timeframes
- Monitor patient progress, symptoms, and response to treatment
- Administer medications and treatments in accordance with provider orders and established standards
- Educate patients and caregivers on disease management, medications, safety, and self-care
- Coordinate care with physicians, interdisciplinary team members, patients, and families
- Maintain timely and thorough clinical documentation in HCHB
- Identify changes in patient condition and communicate concerns promptly
- Deliver compassionate, patient-centered care that promotes safety, independence, and quality of life
