Home Health Registered Nurse (RN) – Bloomington, IN | 13-Week Contract

Make a meaningful difference as a Home Health Registered Nurse serving patients throughout Bloomington and surrounding southern Indiana communities. We are seeking an experienced Home Health RN to provide skilled, compassionate nursing care in patients’ homes and assisted living facilities. This 13-week contract offers a Monday-Friday schedule, a varied caseload, and the opportunity to help patients recover and remain safely in their communities. Home Health RN Job Details

  • Location: Bloomington, Indiana
  • Weekly Estimated Pay: $1,830-$1,930
  • Assignment Length: 13 weeks
  • Start Date: October 26, 2026
  • Schedule: Monday-Friday, 5 shifts per week
  • Shift Duration: 8-hour days, generally 8:00 a.m.-5:00 p.m.
  • Typical Caseload: Approximately 5-8 patients per day
  • Travel: Home visits within a territory that may extend up to 75 miles from the assigned care center
  • On-Call: Approximately one Monday-Thursday night rotation every four weeks
  • Productivity: Approximately 30 weekly productivity points, generally equating to a full-time workload
  • Hours: Contract hours are not guaranteed; additional nearby assignments may be available based on patient census

Home Health Registered Nurse Requirements

  • Current Registered Nurse license in hand; pending licenses are not accepted
  • At least 1 year of recent adult home health or hospice nursing experience
  • Current CPR/BLS certification
  • Recent proficiency with OASIS; experience within the last 6 months is preferred, otherwise retraining may be required
  • Recent proficiency with the HomeCare HomeBase electronic medical record; experience within the last 6 months is preferred, otherwise retraining may be required
  • Valid driver’s license, reliable personal transportation, and required auto insurance
  • Willingness and ability to travel between patient homes and attend periodic office meetings
  • Ability to complete all clinical documentation within 24 hours of each patient visit
  • Flexibility to accept scheduled patient visits throughout the assigned territory and participate in on-call rotations

Home Health Nursing Responsibilities

  • Provide skilled nursing care in patients’ homes and assisted living facilities in accordance with provider orders, care plans, and agency policies
  • Complete comprehensive assessments, admissions, start-of-care visits, wound care, follow-up visits, and other skilled home health services
  • Apply the nursing process to assess patient needs, develop and implement care plans, evaluate outcomes, and coordinate changes in care
  • Use OASIS and the HomeCare HomeBase system accurately and efficiently to support continuity of care and timely documentation
  • Coordinate care with providers, therapists, aides, families, and other members of the interdisciplinary healthcare team
  • Educate patients and caregivers regarding medications, disease management, safety, treatment plans, and when to seek additional care
  • Directly supervise and evaluate Home Health Aides and Licensed Practical/Vocational Nurses as assigned
  • Complete documentation within required timeframes and maintain accurate records of patient progress and services provided
  • Participate in office meetings, territory coordination, productivity planning, and assigned on-call coverage
