Home Health RN Job Details

• Location: Vero Beach, Florida, with coverage throughout Port St. Lucie, Fort Pierce, and Martin County
• Healthcare Setting: Home health
• Weekly Estimated Pay: $2,120–$2,230
• Start Date: October 19, 2026
• End Date: December 31, 2026
• Contract Duration: Approximately 10 weeks
• Schedule: 40 hours per week; 8-hour day shifts scheduled approximately 8:00 a.m.–4:30 p.m.
• Shift Duration: 8 hours
• On-Call and Weekend Requirement: None

Home Health Registered Nurse Requirements

• Home health nursing experience
• Associate degree in nursing or equivalent nursing education
• Current BLS/CPR certification
• Homecare Homebase (HCHB) experience preferred

Home Health Nursing Responsibilities

• Conduct skilled nursing start-of-care visits in patients’ homes
• Complete OASIS assessments and develop individualized plans of care
• Assess patient conditions, identify changes in health status, and communicate clinical findings to the care team and providers
• Provide patient and caregiver education regarding medications, disease management, safety, and ongoing care needs
• Coordinate care with physicians, therapists, aides, and other members of the interdisciplinary team
• Maintain accurate, timely, and compliant clinical documentation
• Manage a field-based schedule across assigned service areas while meeting weekly productivity expectations of approximately 30 points
• Support safe transitions of care and promote positive patient outcomes in the home setting
