Home Health RN Job Details

• Location: Vero Beach, Florida, with coverage in Vero Beach, Fort Pierce, and Sebastian
• Healthcare Setting: Home health care
• Weekly Estimated Pay: $2,120-$2,230
• Start Date: October 19, 2026
• Assignment End Date: December 31, 2026
• Nursing Schedule: Monday-Friday, approximately 8:00 a.m.-4:30 p.m.
• Shift Duration: 8-hour shifts
• Hours: 40 hours per week
• Weekend and On-Call Requirement: None
• Travel: Mileage between patient visits is reimbursed

Home Health Registered Nurse Requirements

• Active Registered Nurse (RN) license eligible for practice in Florida
• At least 1 year of professional nursing experience
• Previous home health nursing experience required
• Hands-on experience with HCHB documentation and workflows required
• Experience completing OASIS assessments required
• Associate degree in nursing or higher
• Current BLS/CPR certification
• Strong clinical judgment, organization, communication, and nursing documentation skills
• Ability to work independently while coordinating care with patients, families, and the broader healthcare team
• Valid driver's license and reliable transportation for travel between patient visits

Home Health RN Responsibilities

• Perform comprehensive nursing assessments and develop individualized plans of care for patients in their homes
• Complete accurate and timely OASIS assessments and compliant clinical documentation
• Provide skilled nursing interventions based on patient needs and established plans of care
• Monitor patient progress, identify changes in condition, and communicate concerns promptly to the appropriate members of the care team
• Educate patients and caregivers on medications, disease management, treatment plans, safety, and symptom monitoring
• Coordinate home health care with physicians, therapists, social workers, caregivers, and other healthcare professionals
• Perform or support clinical tasks such as IV care, wound-vac management, and blood draws when indicated and within scope of practice
• Maintain complete, compliant, and patient-centered patient records using HCHB
• Manage an efficient daily home visit schedule while meeting productivity expectations and maintaining a high standard of skilled nursing care
