Home Health Registered Nurse (RN) – Homecare Contract in Barre, Vermont

Home Health RN Job Details

• Weekly Estimated Pay: $2,130–$2,250
• Job Location: Barre, Vermont
• Healthcare Setting: Home health and community-based care
• Schedule: Monday–Friday, day shift
• Shift Duration: 12-hour days
• Guaranteed Hours: 36 hours per week
• Assignment Duration: Approximately 26 weeks
• Assignment Dates: November 24, 2026–May 25, 2027

Registered Nurse Job Requirements

• Active Registered Nurse (RN) license eligible for practice in Vermont
• At least 1 year of professional nursing experience; home health, hospice, care management, or community-based nursing experience is preferred
• Strong clinical assessment, care planning, documentation, and patient education skills
• Ability to work independently in patients’ homes and manage a daily caseload
• Reliable transportation and the ability to travel throughout the assigned service area
• Excellent communication and collaboration skills with patients, families, providers, and interdisciplinary care teams

Home Health Nursing Responsibilities

• Provide individualized, goal-oriented skilled nursing care in accordance with physician orders and established plans of care
• Complete comprehensive in-home patient assessments and monitor changes in condition
• Develop, implement, and update nursing plans of care using the nursing process
• Serve as case manager when the RN is the lead discipline, coordinating services and promoting continuity of care
• Educate patients and caregivers on medications, treatments, disease management, safety, and self-care
• Communicate patient progress and clinical concerns to providers and members of the interdisciplinary team
• Maintain timely, accurate, and compliant clinical documentation
• Advocate for patients and support high-quality care in the home environment
