Description

Minimum 1-year recent Home Health/Hospice experience / CERTIFICATIONS – BLS (Required) / LICENSE – Active RN license required at submission / Schedule: Days, 8s/10s, 40 hrs/week, 13 weeks (8a–5p + on-call rotation evenings/weekends; holiday coverage required) / Unit: Home Health & Hospice (Adults, Geriatrics, Peds) / EMR: HomeCare HomeBase / Responsibilities: Initial & ongoing assessments, care plan development, medication administration, case management, physician communication, discharge planning, supervise home health aides / Coverage Area: 60-mile service radius (mileage built into bill rate) / Additional: Current Driver’s License & Auto Insurance required, No clinical involvement, RTO required upfront – no additional RTO after submission / Radius Rule: 50 miles (No CommonSpirit/CHI/Dignity employment within past year) / PAY IS NEGOTIABLE!
