Minimum 2+ years recent Home Health RN experience / CERTIFICATIONS – BLS Required + Valid Driver’s License & Insurance Required / LICENSE – Active RN License Required / Schedule: Days, 8-hour shifts, 40 hrs/week, every other weekend (flexible: Mon–Fri or Tue–Sat preferred), 13 weeks / Setting: Home Health / EMR: Cerner (HomeCare HomeBase preferred) / Caseload: ~5 visits/day (point-based system: SOC 2.5, Recert 1.5, ROC 1.75, Routine 1 pt) / Patient Types: Adult patients (no peds), post-discharge high-risk (CHF, COPD, wounds, wound vacs, IV/PICC lines) / Skills: Home visits, patient assessment, wound care, IV therapy, patient education, care coordination / Travel: Required (mileage reimbursed per GSA guidelines) / Call: No call required / Support: Home Health Aide available, manager & admin support / Scheduling: Dynamic scheduling (updated daily), flexible workweek options / Holiday: Assistance needed for Thanksgiving and/or Christmas / Additional: No floating outside Home Health, professional scrubs (any color) / PAY IS NEGOTIABLE!
