Licensed Practical Nurse – Skilled Nursing Facility

Minimum 1 year of recent LTC/SNF experience required. Must have experience in large long-term care facilities and must have completed prior travel assignments successfully (no first-time travelers).

Education:

Graduate of an accredited Practical Nursing program

Certifications / Licensure:

Active LPN/LVN State License (state where facility is located) – Required
BLS (AHA) – Required
Mandatory Reporter Training – Adult – Required
Iowa Dementia Training (8 hrs annually) – Required

Clinical Skills:

Experience caring for geriatric and long-term care residents
Knowledge of memory care and dementia patient management
Ability to work with IV lines (IV certification required for LPNs if applicable)
Experience with medication administration and patient monitoring
Familiarity with PointClickCare EMR

Schedule:

Shift: Nights
Shift Length: 12 hours
Schedule: 3 × 12-hour night shifts per week
Assignment Length: 13 weeks
Weekend Requirement: Every other weekend

Other Notes:

Floating between halls/units within the facility required
Dress code: Any scrub color (no sweatshirts or t-shirts)
Must assist with resident transport to hospital departments (lab, radiology, etc.) when needed.
