Minimum 1+ year recent RN experience in Home Health required (plus 1 year of Med-Surg hospital experience) / CERTIFICATIONS – BLS (AHA) required / LICENSE – Active Kentucky RN License required / Schedule: Days, 8-hour shifts, 40 hrs/week, 13 weeks (5x8s) with weekend and holiday rotation / Setting: Post-Acute – Home Health / EMR: Homecare Homebase (HCHB) – Homecare Homebase experience is mandatory (no exceptions) / Skills: Home health case management, Start of Care (SOC), recertifications, routine home visits, comprehensive patient assessments, medication management, wound care, patient education, documentation, and care coordination required / Patient Population: Adult home health patients only / Coverage Area: Nelson, Spencer, and southern Bullitt counties (no mileage reimbursement) / Productivity: Approximately 6.5 points per day, managing a caseload of approximately 30 patients / Weekend Requirement: Rotating weekends (typically 8:00 AM–5:00 PM based on staffing rotation) / Holiday Requirement: Required / Call Requirement: None / Support: Clinical Manager/Educator available 24/7 for emergencies and 8:00 AM–5:00 PM for routine support / Orientation: Two weeks with a preceptor, including supervised home visits with a work coach / Equipment: Phone and tablet provided / Dress Code: Scrubs (any color) / Additional: Candidates must have both recent Home Health experience and Homecare Homebase (HCHB) experience—no exceptions. This position requires independent field work, strong organizational skills, and the ability to complete SOCs, recertifications, and daily visits efficiently.
