Description
RN – Home Health (Kentucky) / HCHB (Homecare Homebase) and Home Health experience are hard requirements / KY RN License must be in hand at submission / 1+ year Med/Surg hospital experience required / 5x8-hour shifts, typically 8 AM–5 PM / No on-call requirement / Adult patients only, case managing approximately 30 patients with 6.5 points/day / Responsible for Start of Care, recertifications, and daily visits / Weekend and holiday rotation required / 2 weeks of orientation with a work coach / Phone and tablet provided / Scrubs required with no color restriction / No clinical involvement.
