Local Rates apply within 80 miles
1. Will Therapist need to cover SNF and/or Home Health? If so, which locations? Coverage at Grand Island, Hastings and Home Health 2-3 days per week and Kearney SNF and HH 2-3 days per week
2. Will Therapist need to supervise Assistants? NA
3. Software used: Net Health and HCHB
4. Will train New Graduate? Yes - we have a therapy supervisor onsite and our clinical specialist and director are also available.
5. Productivity expectations? SNF: 82% Therapists (evaluation, group, and concurrent time is counted in full); HH 90% which includes drive time and documentation time
6. Guarantee: 40 (or 36); M-F or able to do a shifted schedule T-Sa or Su-Th
7. Targeted State Date: as soon as possible - the SLP in Kearney is on maternity leave - once she returns the time will be fully at Grand Island and Hastings SNF and HH
8. Mileage and drive time billable? Paid between locations and from pt to pt in HH
Essential Duties and Responsibilities:
Administers and performs therapeutic interventions with clients. Assesses clients’ needs and develops plans for therapy that meets identified needs. Safeguards clients’ property. Provides clients with opportunities for independence and choice consistent with client plan of care. Maintains clients’ self-esteem, privacy and confidentiality of personal information. Assists to keep clients’ living areas pleasant and orderly. Documents observations and services provided. Notifies physician of changes; processes orders provides direct consultation with physician and/ or interdisciplinary team members. Receives and shares information. Trains clients/ family members/ staff on interventions, equipment. May utilize and supervise therapy assistants as outlined by Federal and State regulations and documents appropriately in the clinical record. Utilizes appropriate bag techniques and standard precautions. Follows infection control and safety control standards when using equipment.
Basic Responsibilities:
This position will be held accountable for complying with all related laws, regulations, company policies and procedures pertaining to his or her position and for fulfilling his or her obligations under the Facilities Corporate Compliance Program.

Qualificiations:
Education Experience Licensure Certification:
Completion of an accredited speech/ language pathology program preferred. Prior work experience in a rehabilitation/ geriatrics setting preferred. Certification for the state of practice
Skills/Abilities:
Basic speech/language pathology skills. Must be at least 18 years of age.
Knowledge:
Basic ability to communicate and comprehend.
