Description
- Review and prioritize patients, including new admissions.
- Complete patient assessments and care plans in Epic.
- Participate in daily Care Management touchpoints and CAPP meetings.
- Collaborate with Social Work, CMA, Utilization Management, and the care team.
- Identify barriers to discharge and update discharge plans.
- Assist with determining Expected Discharge Date (EDD).
- Attend weekly Complex Care Meetings (CCM) and present complex cases.
- Identify high-risk patients and escalate cases as needed.
- Follow up on action items and coordinate solutions for complex patients.
