About Facility: o Home health, rural territory ● Unit Information: o Average 6 patients per day. Visits, evaluations, admissions and discharges ● Patient Types/Common Diagnoses: o Geriatric ● Patient Ratios: o 2 PTs, 1PT in orientation, 1 PTA ● Required License, National Certification, Certs (BLS, ACLS, etc.), or other position specific requirements such as Drivers License and/or Insurance: o PT license, CPR certification, car insurance ● Technology/Equipment: o EMR: HCHB ● Floating: If yes, where? o Branch coverage area (Cleveland, Gaston, Rutherford counties) ● Orientation (facility and unit): o 5 days total orientation ● Scheduling: o Weekend rotation: no o On call? If so, what is that schedule? no o Holiday Expectations: as needed o Should approval of RTO go through manager or will you allow the interviewing team (if applicable) to approve time off on your behalf? manager o Shift times: 8-5 o Schedule cycle: M-F ● Other notes: o Scrub Color: light gray
