Job Summary:

Provides holistic, patient- and family-centered care focused on individualized goals.
Collaborates with patients, families, and healthcare team members to develop and document comprehensive care plans.
Implements and adapts care plans using age- and culturally-appropriate nursing interventions.
Evaluates patient and family progress toward goals, revising interventions as needed and documenting responses.
Assesses, develops, and implements individualized patient/family educational plans, addressing learning needs and barriers.
Evaluates effectiveness of patient/family education and revises teaching plans as necessary.
Identifies and addresses communication barriers, adapting care for diverse socio-cultural needs.
Advocates for patients and families, supporting their participation in decision-making and upholding their rights.
Accurately documents patient and family care needs per departmental guidelines and policies.
Delegates and assigns tasks to appropriate healthcare team members based on scope of practice and competency.
Coordinates and collaborates with the healthcare team to optimize patient and family well-being and care delivery.
Communicates and works with ancillary staff to improve patient progress and workflow.
Requires at least three years of recent RN experience, including at least two years in a specialty area.
Must hold a current California RN license and BLS certification (AHA).
