Description
Job Summary (List Format):
- Provide hospice care and support to patients and families in the final stages of illness within home and facility settings (including private residences, skilled nursing facilities, assisted living, and adult foster homes).
- Deliver intermittent skilled nursing visits, pain and symptom management, medical social services, counseling, spiritual support, and bereavement support.
- Coordinate with hospice aides, trained volunteers, and other interdisciplinary team members to deliver comprehensive care.
- Assess patient and family needs, develop care plans, and provide emotional and psychosocial support.
- Offer guidance on medications, equipment, and available hospice resources as appropriate.
- Utilize personal vehicle to travel to patient locations across multiple counties (Linn, Benton, Marion, Polk, Lincoln, and Tillamook).
- Maintain accurate documentation and comply with all legal and organizational requirements.
- Preference given to candidates with previous hospice or hospital/medical social work experience.
- Must hold Oregon LMSW or CSWA licensure; Oregon LCSW preferred.
- Maintain valid driver’s license, auto insurance, and vehicle in good working order.
- Current Healthcare Provider Level BLS certification required.

Med Ninjas Job ID #15259. Pay package is based on 8 hour shifts and 40 hours per week (subject to confirmation) with tax-free stipend amount to be determined.
