Job Summary: Licensed Practical Nurse (LPN) – Long-Term Care

- Provide direct nursing care to residents in a long-term care or skilled nursing facility.
- Administer medications and treatments as prescribed and in accordance with facility protocols.
- Monitor residents’ vital signs and overall condition, reporting significant changes as needed.
- Perform nursing assessments and communicate findings to RNs, physicians, or appropriate healthcare providers.
- Assist residents with activities of daily living (ADLs) such as bathing, dressing, grooming, toileting, and feeding.
- Carry out wound care, dressing changes, catheter care, and other nursing procedures within the LPN scope of practice.
- Maintain accurate and timely documentation in the electronic medical record (EMR).
- Collaborate with interdisciplinary team members including RNs, CNAs, physicians, therapists, and social workers.
- Educate residents and families on medications, treatments, safety, and ongoing care needs.
- Follow and contribute to individualized care plans based on residents’ evolving requirements.
- Monitor and address signs and symptoms of infection, falls, pressure injuries, and other complications.
- Respond promptly to emergencies and significant changes in residents’ clinical status.
- Assist with the processes for admissions, transfers, and discharges as needed.
- Adhere to infection-control protocols and comply with facility, state, and regulatory standards.
- Promote resident dignity, safety, comfort, and independence while delivering compassionate care.
