RN – Hospice On-Call | Dallas, TX

Position: Hospice On-Call RN
Location: Dallas, TX Area
Schedule: 7 On / 7 Off
Hours:

• Monday–Friday: 5:00 PM – 8:00 AM
• Saturday/Sunday: 24-hour on-call coverage, 8:00 AM – 8:00 AM
• One week on / one week off
• Paid: Standard 40 hours/week

Required Experience

• Strong Hospice Nursing experience required
• Previous Hospice On-Call experience required
• Hospice Admissions experience required
• Experience with HCHB (Homecare Homebase) EMR required

Key Responsibilities

• Assess patients’ physical, psychosocial, environmental, safety, and developmental needs and individualize plans of care.
• Perform comprehensive nursing assessments and develop appropriate goals and interventions.
• Assess and manage pain and other symptoms while developing individualized comfort-care plans.
• Anticipate, prevent, and treat symptoms and complications associated with end-of-life care.
• Provide compassionate care focused on comfort, dignity, and quality of life for patients and families.
• Participate in interdisciplinary team (IDT) meetings to establish, review, and revise patient care plans.
• Maintain accurate and up-to-date electronic medical records, including assessments, interventions, goals, outcomes, and changes in condition.
• Educate patients, families, and caregivers regarding medications, treatments, expected effects, side effects, and appropriate interventions.
• Coordinate patient and family services with the interdisciplinary care team and other healthcare professionals.
• Support patients and families while respecting their unique spiritual, cultural, and personal beliefs.
• Provide guidance and support during the dying process and assist families with effective grieving.
• Maintain the dignity, comfort, and safety of patients receiving end-of-life care.
• Provide holistic, family-centered hospice care across treatment settings.
• Ensure documentation and care comply with applicable regulations, certification standards, legal and ethical requirements.
• Provide care consistent with National Hospice and Palliative Care Organization (NHPCO) standards of practice.
