Description
Position Overview
The Licensed Practical Nurse (LPN) will provide direct nursing care to residents in a Long-Term Care (LTC) setting. The LPN will work closely with residents, families, physicians, and the healthcare team to ensure safe and quality patient care.
Job Responsibilities
- Provide direct nursing care to residents in a long-term care facility.
- Administer medications and treatments as ordered.
- Monitor and document residents' health conditions and changes in status.
- Perform assessments and communicate significant changes to the appropriate healthcare provider.
- Provide wound care and other treatments as required.
- Assist with activities of daily living and resident care.
- Monitor vital signs and maintain accurate clinical documentation.
- Follow infection control and safety procedures.
- Collaborate with RNs, CNAs, physicians, and other healthcare professionals.
- Respond appropriately to emergencies and changes in residents' conditions.
- Maintain accurate and timely medical records.
- Provide compassionate, patient-centered care to residents.
Required Qualifications
- Active LPN/LVN license in the applicable state.
- LTC/SNF experience preferred or required based on facility requirements.
- Current BLS certification.
- Strong clinical, communication, and organizational skills.
- Ability to work effectively in a fast-paced healthcare environment.

- Provides care to geriatric and medically complex residents.
- May include day, evening, night, weekend, and holiday shifts depending on facility needs.
