Description
Social Worker – Hospital / Inpatient
Job Summary
Provides therapeutic intervention and social work services to patients and families to support comprehensive, integrated, and uninterrupted care during hospitalization and transition to community-based care.
Job Responsibilities
- Conduct psychosocial assessments to identify patient and family needs related to medical conditions and social determinants of health.
- Interview patients, family members, and significant others to assess needs and develop appropriate interventions and treatment goals.
- Participate in multidisciplinary rounds and provide psychosocial input regarding patient care and discharge planning.
- Provide psychosocial support to patients and families during adjustment to illness, hospitalization, and life changes.
- Serve as a patient and family resource and advocate, including support with Advanced Care Planning.
- Collaborate with the healthcare team regarding inpatient needs, post-hospital services, and continuity of care.
- Coordinate comprehensive discharge planning, including referrals to LTC/Custodial Care, Acute Rehab, Sub-Acute Rehab/SNF, LTAC, Dialysis, Hospice, and homelessness resources.
- Connect patients and families with community and governmental resources, including financial assistance, food benefits, transitional housing, and other support services.
- Maintain accurate documentation of psychosocial assessments, patient contacts, family interventions, and discharge planning in accordance with hospital, state, federal, and Joint Commission requirements.
- Maintain confidentiality and professional standards while participating in program planning, quality improvement, and development of community social service networks.
Required Skills
- Excellent communication, negotiation, and conflict-resolution skills
- Strong knowledge of psychosocial services and healthcare resources
- Knowledge of state and federal regulatory standards related to Social Work
- Experience with clinical performance improvement and rapid-cycle change
- Ability to collaborate effectively with multidisciplinary healthcare teams
- Strong discharge planning and care coordination skills
Education
- Master of Social Work (MSW) from an accredited school of Social Work
Experience
- Previous medical hospital social work experience strongly preferred
- Hospital/inpatient adult discharge planning experience required

- Schedule: 8:00 AM – 4:00 PM
- License: Must have an active/valid New Jersey Social Work license at the time of submission.

Med Ninjas Job ID #16940. Pay package is based on 8 hour shifts and 40 hours per week (subject to confirmation) with tax-free stipend amount to be determined.
