Job Summary (List Format) – Clinical Pharmacist Disease Specialist (Amb HIV Hep C):

- Provides clinical leadership in pharmaceutical care for HIV and Hepatitis patients.
- Acts as a resource for outpatient and inpatient clinicians regarding HIV and/or Hepatitis pharmacotherapy.
- Collaborates with Inpatient Infectious Diseases Clinical Pharmacist to ensure smooth transition of HIV patients from hospital to clinic.
- Develops and optimizes patient-specific pharmacotherapeutic regimens via collaborative practice agreements.
- Conducts drug utilization reviews, monitoring for drug interactions and adverse events.
- Works to minimize polypharmacy and creates patient medication action plans with self-management goals.
- Serves as a preceptor for pharmacy students or residents, participating in both didactic and experiential education.
- Functions as an integral member of the multidisciplinary care team within the Medical Home model.
- Provides direct patient care onsite, seeing approximately 14 patients per day as a mid-level provider.
- Maintains compliance with Texas State Board of Pharmacy licensure and BLS requirements.
