Description

Job Summary (Social Worker - Montrose)

- Serve as a member of an interdisciplinary care team (social workers, chaplains, nurses, physicians, CNAs) focused on improving patients’ and families’ quality of life.
- Provide compassionate support, education, and advocacy to patients and families in Montrose County, primarily in home and community-based settings.
- Assess patient and family needs, and connect them with appropriate resources and services to honor their individual goals and wishes.
- Deliver care in collaboration with colleagues in nursing homes, assisted living, and other residential facilities when applicable.
- Foster strong relationships with patients, families, and team members through excellent interpersonal and communication skills.
- Uphold and promote HopeWest’s core values and reputation within the community.
- Participate in ongoing education, volunteerism, and philanthropy to enhance patient care and community impact.
- Support elderly and aging populations, with a preference for experience in geriatric or healthcare social work.
- Act as an ambassador for HopeWest’s mission to positively change the experiences of aging, illness, and grief for patients and families.

Med Ninjas Job ID #17578. Pay package is based on 8 hour shifts and 40 hours per week (subject to confirmation) with tax-free stipend amount to be determined.
