Social Worker – Acute Care Case Management:

Job Description

• Start Date: ASAP
• Experience Required: Minimum 2 years of Acute Care Hospital Case Management experience
• First-Time Travelers: Not accepted
• Patient Ratio: Approximately 1:35
• Weekend Requirement: Every other weekend
• Holiday Requirement: Must be available for holiday coverage
• CA License: Preferred
• Pending License: Accepted
• RTO: No more than 7 days
• Facility Type: Acute Care Hospital
• Dress Code: Scrubs or Business Casual

Key Responsibilities

• Provide case management and social work services to patients in an acute care hospital setting.
• Focus on discharge planning, including assessing patient needs and coordinating safe transitions of care.
• Coordinate with physicians, nurses, rehabilitation teams, insurance providers, and community resources.
• Monitor patient progress and identify barriers to discharge.
• Develop appropriate discharge plans based on the patient's clinical, psychosocial, and social needs.
• Provide referrals to home health, skilled nursing facilities, rehabilitation, hospice, behavioral health, and community resources as appropriate.
• During weekend coverage, monitor up to two units and prioritize discharge planning and urgent case management needs.
• Complete timely documentation and maintain accurate patient records.
• Assist patients and families with understanding available resources, post-acute care options, and discharge instructions.
• Collaborate with multidisciplinary teams to facilitate efficient and safe patient transitions.
