Position: LPN Case Manager
Employment Type: Full-Time
Guaranteed Hours: 40 hours/week
Schedule: Monday–Friday, 8:00 AM–4:30 PM
Weekend: No

Home Base: Riedman Administrative Campus, Rochester, NY
This is a mobile care management position. The LPN may be required to travel during the workday to patient homes, hospitals, and provider offices. Reliable transportation is required. Patients will not be transported by the LPN.
Position Overview
The LPN Case Manager will support the Transitions of Care Program, which focuses on reducing hospital readmissions, particularly within 30 days of discharge.
The LPN will work under the direction of an RN Care Manager to help implement individualized patient care plans. This is primarily a care coordination, telephonic, and case management role—not a hands-on nursing position.
The LPN will work as part of a multidisciplinary team that may include:
- RN Care Manager
- Social Worker
- Pharmacist
- Physicians/Providers
- Other healthcare professionals
Key Responsibilities
- Assist the RN Care Manager with implementing the plan of care.
- Maintain frequent contact with assigned patients.
- Conduct screening and follow-up calls.
- Schedule and coordinate patient appointments.
- Identify and triage barriers to care.
- Monitor biometric data and patient status.
- Provide and reinforce patient education.
- Educate patients on medication compliance and appropriate medication use.
- Attend patient visits when needed.
- Conduct home visits when required.
- Visit hospitals or provider offices as needed.
- Connect patients with appropriate community resources.
- Assist patients with accessing healthcare and social services.
- Participate in virtual multidisciplinary care team rounds.
- Provide patient updates during care team meetings.
- Complete required screening and scoring documentation.
- Document all patient interactions accurately in the EHR.
- Assist with reassessment of patient goals throughout the 30-day program.
- Help determine whether patients should be closed from the program or transitioned to long-term management.
Caseload
The LPN will manage a rolling caseload of approximately 25–30 patients, with approximately 30–40 rotating patients per month.
Patient Population
Adult and geriatric patients with complex medical needs and multiple comorbidities.
Experience or clinical knowledge with the following is preferred:
- CHF
- COPD
- Multiple complex comorbidities
- Care transitions
- Care coordination
- Community resources
Important Role Details
- No hands-on nursing care
- Primarily remote/telephonic care coordination with occasional in-person visits
- Travel may be required to patient homes, hospitals, and provider offices
- Reliable transportation is required
- LPN will not transport patients
- Must be comfortable working independently
- Strong communication and documentation skills are essential
- Must be flexible and able to manage changing priorities throughout the day
- Community resource knowledge is preferred
Floating
The LPN may be required to work at or travel between:
- Rochester General Hospital
- Unity Hospital
- Patient homes
- Provider offices
- Riedman Administrative Campus
Ideal Candidate
The ideal candidate is an LPN with strong care coordination, communication, organization, and patient education skills who is comfortable working independently in a mobile case management environment.
This position is best suited for an LPN interested in care management/care coordination rather than traditional bedside or hands-on nursing.
