Job Summary:

- Position: Licensed Practical Nurse (LPN), Full-Time, 40 hours/week (Monday–Friday, 8:00 a.m.–4:30 p.m.)
- Patient Focus: Adult/Geriatric patients, primarily in the Transitions of Care program.
- Main Responsibilities:
- Support the Transitions of Care program to reduce 30-day readmission rates.
- Work under the direction of the RN Care Manager.
- Non-hands-on nursing role (no direct patient care).
- Assist with implementing patient care plans.
- Schedule appointments and follow-up visits.
- Monitor and record biometric data.
- Provide and reinforce patient education (e.g., medication adherence, disease management).
- Attend patient visits (home, hospital, office) as needed.
- Conduct home visits as required.
- Connect patients with community resources and services.
- Maintain frequent patient contact (phone/virtual/in-person).
- Complete assigned tasks as directed by the RN.
- Participate in multidisciplinary care team rounds and provide patient updates.
- Manage a rolling caseload of 25–30 patients monthly.
- Document all activities in the Electronic Health Record (EHR) clearly and concisely.
- Patient Diagnoses: Primarily CHF, COPD, and complex comorbidities.
- Work Environment: Combination of remote, hospital, patient home, and office settings; not a hands-on clinical role.
- Team Structure: Multidisciplinary team — RN, Social Worker, Pharmacist, and other professionals.
- Special Requirements:
- Minimum 1 year of Home Health/Mobile Case Management experience.
- BLS certification.
- Strong communication and documentation skills.
- Ability to work independently and adapt to changing situations.
- Reliable transportation required; travel is necessary (but no patient transport).
- Local knowledge of community resources preferred.
- Experience with care coordination and chronic disease management preferred.
- Caseload: 30–40 rotating patients per month, typically 30-day program with reassessment for long-term management.
