Description

Job Summary 

Position: Licensed Vocational Nurse (LVN) – Skilled Nursing Facility (SNF)
Reports to: Director of Nursing (DON)
Dress Code: Caribbean Blue scrubs required

### Key Responsibilities

- Provide individualized nursing care to residents in collaboration with physicians, nursing staff, families, and other departments.
- Conduct patient assessments, prioritize care, and promptly notify charge nurse/physician of abnormal findings.
- Develop and implement care plans tailored to each patient, collaborating with the healthcare team.
- Administer medications and treatments as ordered, following facility protocols and documenting appropriately.
- Perform nursing procedures (e.g., EKGs, blood draws, IV starts), ensuring accuracy and infection control.
- Educate patients and families regarding care, treatments, and health maintenance.
- Respond to emergencies, initiate resuscitation protocols, and document all actions taken.
- Maintain accurate documentation of assessments, interventions, and patient progress.
- Report patient conditions and changes to appropriate personnel during each shift.
- Assist with direct patient care and supervise CNA tasks, ensuring completeness and documentation.
- Support other departments and coordinate patient transfers and diagnostic procedures as needed.
- Orient and mentor new staff and participate in ongoing education and staff meetings.
- Uphold safety and infection control standards at all times.
- Maintain confidentiality regarding patient, staff, and facility information.
- Demonstrate professional behavior, effective communication, organizational skills, and time management.
- Adhere to facility policies, dress code, and regulatory requirements.

### Qualifications

- Current California LVN license in good standing
- Current BLS/CPR certification
- Good physical and mental health; stamina for walking, bending, lifting, and performing manual skills
- Ability to tolerate and appropriately respond to challenging patient behaviors
- Strong assessment and emergency response skills
- Excellent interpersonal, communication, and problem-solving skills
- Dependable, adaptable, and able to work collaboratively as a team member
- Interest in ongoing education and professional development
- Ability to use computers and Microsoft Office applications
- Must wear Caribbean Blue scrubs as uniform

Med Ninjas Job ID #18482. Pay package is based on 12 hour shifts and 36 hours per week (subject to confirmation) with tax-free stipend amount to be determined.
