Job Summary (List Format) – Community Health LPN (Home Health, Berkshire Medical Center)

General Overview:
- Provides and documents nursing care for sick and injured patients at home.
- Promotes, maintains, or restores health, prevents illness, and supports rehabilitation.
- Collaborates with healthcare providers, professionals, and community representatives to coordinate at-home care.
- Evaluates patient and family response to interventions.
- Functions independently under general guidance with adherence to protocols.
- Requires independent judgment, critical thinking, and emotional stability in emergencies.
- Utilizes time management, communication, planning, delegation, and problem-solving skills.

Key Responsibilities:
- Exercise independent judgment in patient care.
- Maintain confidential information and accurate records (verbal and written communication).
- Demonstrate interpersonal skills with patients and team members.
- Maintain flexibility regarding work hours and be willing to accept “on call” responsibilities.
- Drive an automobile in inclement weather to provide home care.
- Adhere to BHS Customer Service and Performance Standards and Patient’s Rights Policy.
- Exhibit dependability and meet committed work schedules.

Required Skills & Experience:
- Minimum 1 year of acute nursing care (hospital-based preferred).
- Experience in home health required.
- Certified in Home Health, with OASIS proficiency highly preferred.
- Infusion therapy experience preferred.
- Ability to work independently with minimal supervision.
- Familiarity with home care reimbursement and regulatory guidelines.
- Flexibility in assignments and experience with self-scheduling preferred.
- Must have a valid driver’s license, insurance, and personal vehicle.

Licensing & Certification:
- Active Massachusetts LPN license (must be uploaded with profile).
- Current BLS certification from the American Heart Association or American Red Cross.

Submission & Profile Requirements:
- Submit agency’s MA Temporary Nursing Service Agency License with candidate profile.
- Candidate’s permanent address and all requested time off (RTO) must be included at submission.
- No consideration for candidates whose permanent residence is within 50 miles of the facility.
- Client prefers traveler housing within 50 miles and a 45-60 minute commute from TOR office in Pittsfield, MA.
- Work history, current Home Health RN skills checklist (within 1 year), and 1 supervisory reference from the past 2 years required.

Schedule & Time Off:
- Requested time off must be submitted with the profile; changes not allowed after submission.
- No more than 5 days of RTO accepted; less is preferred.
- Must be available to work during major holidays (Thanksgiving, Christmas, New Year) per required scheduling blocks.
- Non-billable orientation: 8 hours.
- Holiday and OT multipliers: 1.25%.
