LPN – Long-Term Care (LTC):

Job Title: Licensed Practical Nurse (LPN) – LTC

Shift - 3 PM - 11 PM Every Other Weekend Required

Job Responsibilities

• Provide direct nursing care to residents in a long-term care or skilled nursing facility.

• Administer medications and treatments as prescribed by physicians.

• Monitor vital signs and assess residents’ health conditions.

• Report changes in residents’ conditions to the RN or healthcare provider.

• Assist with wound care, documentation, and implementation of care plans.

• Collaborate with RNs, CNAs, and other healthcare professionals to ensure quality patient care.

• Maintain infection control, patient safety, and facility standards.

• Support residents with daily care needs while maintaining their dignity and comfort.

Required Qualifications

• Active LPN license in the state of employment.

• Previous LTC, SNF, or nursing home experience preferred.

• Strong medication administration, communication, and documentation skills.

• Ability to work effectively in a team-oriented healthcare environment.
