Job Title: CWHH-LPN-LVN
Location: Louisville, KY
Setting: Home Health / Hospice
Pay Range: $38.29 - $40.20 per hour
Schedule: 36-40 Hours per week (Rotating shifts to meet operational needs)
Contract Length: 06/24/2026 (Temporary)
Candidate Type: Local candidates only – travel stipend not available
Requirements:
- 2 years of experience as an LPN/LVN
- Qualified applicants MUST have a current Nursing License in Kentucky
- Must have a valid driver’s license and current auto liability insurance
- Proof of Hepatitis consent/declination and TB vaccination record
- Ability to use electronic medical charting and time-keeping software (Homecare Homebase)
- Ability to read, write, and speak fluent English
- Good and regular attendance
- Ability to recognize unsafe working environments and notify supervisor immediately
- Participate in annual in-service and performance improvement programs
- Be willing to assist with end-of-life care for hospice patients
Preferred Skills:
- Experience in home health or hospice settings

Responsibilities:
As a healthcare professional in a home health/hospice setting, you will:
- Assist in providing nursing services under the supervision of an RN, identifying patient needs through observation and documentation.
- Coordinate care and participate in planning to deliver quality, cost-effective patient care.
- Communicate effectively with patients, families, and healthcare team members, documenting all interventions accurately.
- Educate patients and families on self-care techniques and assess their response.
- Provide care using appropriate infection control measures (OSHA standards).
- Support end-of-life care duties as directed by the RN and physician.
- Assist with admitting, discharging, and transferring patients as needed.
- May perform on-call duties to ensure continuous patient support.
