Job Title: LPN : Telehealth
Location: Louisville, KY
Setting: Assisted Living
Pay Range: $33.17 - $34.83 per hour
Schedule: 40 Hours per week (M-F)
Contract Length: Discuss with Recruiter
Candidate Type: Local candidates only – travel stipend not available

Requirements:

• 2 years of experience as an LPN
• Qualified applicants MUST have a valid FL LPN license (compact accepted)
• Must have the ability to take vital signs, clear communication skills, navigate email and smartphone
• Provide care for clients following the client's plan of care at all times while maintaining client's dignity and independence
• Monitoring client's vital statistics and reporting abnormal findings
• Administration of medications, feedings, oxygen, ostomy care as needed per client's plan of care and provided assessments of results
• Complete routines as established by PT, OT, Speech Therapists
• Insure infection control policies are maintained, report communicable diseases or infections, and ensure proper infection control procedures
• Accurately document nursing actions, progress, outcomes, and communication as appropriate
• Order supplies, medications, and feedings as needed to ensure adequate stock
• Ensure all patient needs are met per physician instructions
• Monitor incidents, complete incident reports timely and accurately, and notify appropriate personnel
• Maintain medical equipment and report faulty equipment for repair

Responsibilities: As a healthcare professional in an assisted living facility, you will be responsible for providing compassionate, high-quality care to clients.

• Perform nursing duties as an LPN, including vital signs and medication administration
• Use key skills such as clear communication and electronic documentation to ensure seamless care delivery
• Monitor clients’ health and report abnormal findings using industry-standard keywords
