Job Details:

Responsibilities:

• Perform clinical assessment to identify patient needs and appropriateness of care.
• Document, prepare/review, and complete a Plan of Care (POC), progress notes, phone orders, and other clinical records; revise the POC based on ongoing assessments.
• Collaborate with physicians and interdisciplinary teams to develop and implement treatment plans.
• Plan and provide treatment for neuromuscular, neurological, and orthopedic disorders based on medical referrals.
• Instruct patients and families on transfer techniques, proper use of equipment such as walkers, wheelchairs, braces, and prosthetic devices.
• Utilize therapy equipment tailored to patient diagnoses and needs, and order/adapt rehabilitative equipment as necessary.
• Educate patients and families on therapeutic procedures, considering age, condition, culture, and learning readiness.
• Oversee other team members in performing patient care aligned with patient needs and preferences.
• Apply infection control and universal precautions in all patient care activities.
• Accept clinical assignments consistent with education and competence.
• Perform other related duties as assigned.
