Responsibilities:

• Perform clinical assessments to identify patient needs and appropriateness of care.
• Document, prepare/review, and complete a Plan of Care (POC), progress notes, phone orders, and other clinical records on an ongoing basis. Revise the POC based on evaluation, ongoing assessment data. Work in consultation with physicians and as part of an interdisciplinary team.
• Plan and provide appropriate treatment for neuromuscular, neurological, and orthopedic disorders based on medical referral and therapy assessments.
• Assist/instruct patients and families in transfer techniques appropriate to patient abilities. Instruct on use of equipment such as walkers, wheelchairs, canes, braces, prosthetic devices, and appliances in a safe manner. Utilize appropriate therapy equipment specific to patient diagnosis and needs. Order appropriate rehabilitative equipment to meet patients' needs and adapt equipment, appliances to meet home limitations/requirements.
• Provide patient/family education regarding therapeutic procedures based on the patient's age, condition, culture, and learning readiness.
• Oversee and collaborate with other team members when applicable, ensuring care is consistent with patient needs and desires.
• Apply infection control and universal precautions to prevent contamination and disease transmission during patient care activities.
• Accept clinical assignments that align with education and competence to care for patients.
• Perform other related duties as assigned or requested.
