Job Title: RN: Home Health
Location: Palatka, FL
Setting: Home Health

Schedule: 8-Hour Days (on-call as needed by rotation)
Contract Length: 13 Weeks
Candidate Type: Local and traveler friendly

Requirements:

• 1+ years of experience as an RN
• Qualified applicants MUST have at least 1 year of experience in the Home Health setting
• Valid Registered Nurse license
• Be willing to obtain Florida or compact licensure

Responsibilities:
As a healthcare professional in a Home Health setting...

• Deliver one-on-one, high-touch care to patients in their homes, including RN duties
• Conduct skilled nursing visits and perform assessments
• Create and manage individualized care plans
• Collaborate with families and interdisciplinary providers to improve patient outcomes
