Job Title: CWHH-RN Home Health
Location: 27385 Andrew Jackson Highway East, Delco, NC 28436
Setting: Home health care
Pay Range: $47.07 - $49.42 per hour
Schedule: Hours per week: 40, Shift: 8-hour days (rotating)
Contract Length: 13 weeks
Candidate Type: Traveler friendly – travel reimbursement available

Requirements:

• 2 years of experience as a RN
• Qualified applicants MUST have current NC RN license, CPR certification, and Homecare Homebase & OASIS experience
• Must have reliable transportation and insurance
• Bachelors of Science in Nursing (preferred)
• Demonstrated ability to assess and respond to patient and family needs in a home setting
• Ability to communicate effectively across all organizational levels
• Knowledge of medication administration, venipuncture, IV care, wound care, and medical equipment
• Ability to organize tasks, develop action plans, and handle stressful situations
• Maintain current licensure and meet continuing education requirements
• Be willing to work flexible schedule and travel locally

Responsibilities:
As a healthcare professional in a home health setting, you will:

• RN: Assess patient conditions and develop individualized care plans
• Provide patient education on medications, safety, and self-care practices
• Skillfully manage wounds, administer inhalation therapy, perform phlebotomy, and care for indwelling devices
• Collaborate with interdisciplinary teams to coordinate patient care and facilitate referrals
• Maintain accurate documentation and ensure compliance with third-party payers
• Answer on-call duties and serve as a liaison for patients and families
