Job Details:
- Job Title: Home Health LPN

- Pay Range: $1519.93 - $1595.93 gross per week
- Schedule: 8:00AM-5:00PM
- Contract Length: 13 weeks

Responsibilities:
- Provide comprehensive nursing care to patients in a home health setting.
- Conduct thorough assessments of patient health status and develop individualized care plans.
- Collaborate with interdisciplinary teams including physicians, therapists, and social workers.
- Educate patients and families on health management and resources.
- Document patient progress and maintain detailed medical records.
