Responsibilities:

• Assess home health patients in stable and emergent situations to identify physical, psychosocial, and environmental needs.
• Develop, implement, and evaluate the Plan of Care (POC) to ensure quality and continuity of care.
• Collect data on factors influencing patient health and incorporate into the POC.
• Maintain accurate patient records reflecting all visits, contacts, treatments, and changes.
• Provide care using appropriate infection control measures, ensuring staff and patient safety.
• Educate patients and families on care, medications, safety, and self-care practices.
• Supervise and provide clinical guidance to HHA/STNA/LVN team members.
• Assess and manage patient pain, educating on pain management strategies.
• Arrange referrals to appropriate service agencies as needed.
• Monitor cases for third-party payer compliance.
• Perform on-call duties and respond to patient needs during assigned on-call shifts.
• Communicate effectively with patients, families, healthcare team members, and providers.
• Attend interdisciplinary team meetings and perform other related duties as required.
