Job Title: CWHH-RN Home Health
Location: 625 Kenmoor Avenue Southeast, Grand Rapids, MI 49546
Setting: Home Health
Pay Range: $2,142.20 - $2,249.31 weekly
Schedule: 36-40 Hours per week ( rotating shifts to meet operational needs)
Contract Length: Discuss with Recruiter

Requirements:
- 2 years of experience as a RN
- Qualified applicants MUST have at least 2 years of experience in home health nursing
- Qualified applicants MUST have a current state RN license
- Must have Homecare Homebase Software and OASIS experience
- Must have proof of current CPR certification
- Must have valid driver’s license, reliable transportation, and insurance
- Experience with venipuncture, IV care, wound care, medication administration, and use of durable medical equipment
- Bachelors of Science in Nursing preferred

Responsibilities:
As a healthcare professional in a home health setting, you’ll be responsible for assessing patients in their residence, developing and evaluating care plans, providing patient education, supervising health aides, managing pain, and coordinating care with interdisciplinary teams.
- Conduct comprehensive assessments of home health patients to identify needs and develop personalized plans of care (POC).
- Implement and evaluate care plans, ensuring quality and continuity of care while maintaining accurate documentation.
- Educate patients and families on care, medications, and safety measures, promoting self-care practices.
- Supervise and provide clinical direction to home health aides and LVNs.
- Monitor and reassess pain management strategies, educating on techniques.
- Arrange referrals to appropriate services and ensure compliance with payer requirements.
- Perform on-call duties as assigned, providing support to patients and families during emergencies.
- Communicate effectively as a liaison among patients, families, team members, and providers.
- Attend interdisciplinary team meetings and perform other related duties as needed.
