Pay Range: 2013.28 - 2113.94

Contract Length: 13 weeks.

Responsibilities:

Assess home health patients in various situations to identify physical, psychosocial, and environmental needs.

Develop, document, and evaluate the Plan of Care to ensure quality and continuity of care.

Educate patients and families regarding care, medications, safety, and self-care practices.

Supervise and direct home health aide, STNA, or LVN staff to ensure quality service.

Perform on-call responsibilities and provide services to patients as assigned.
