Job Details:
Job Title: CWHH-LPN-LVN
Location: Raleigh, NC
Setting: Home Health
Pay Range: 1484.61 - 1558.84
Schedule: Discuss with recruiter.
Contract Length: 13 weeks.
Candidate Type: Traveler
Requirements:
- Current Nursing License in the Practicing State
- Valid Drivers License
- Current CPR Certification
- Two years experience as an LPN/LVN in a clinical setting, preferably in a home health or hospice setting
- Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing
Responsibilities:
- Administer skilled nursing care to patients in their place of residence as prescribed by the physician and in accordance with the established plan of care.
- Assist in providing nursing services and identifying patients' physical, psycho-social, and environmental needs.
- Participate in planning, coordinating, and implementing patient care in conjunction with the RN.
- Communicate effectively with patients, families, co-workers, and other healthcare professionals.
- Assist patients and families in learning self-care techniques and assess their responses to care.
- Follow infection control measures to protect staff and patients.
- Assist with admitting, discharging, and transferring patients.
- Possible on-call duties.
- Recognize and maintain professional boundaries; report unsafe environments.
- Participate in annual in-service and performance improvement programs.
- Perform other related duties as assigned.
