Job Details:
Job Title: RN Home Health

Responsibilities:
- Assess home health patients in stable and emergent situations to identify physical, psychosocial, and environmental needs.
- Develop, implement, and evaluate the Plan of Care to ensure quality and continuity of care.
- Educate patients and families regarding care, medications, and safety measures, promoting self-care practices.
- Supervise and direct home health aides/stna/lvn to maintain quality of service.
- Perform on-call responsibilities and provide on-call services to patients and families as assigned.
