Job Title: PT Home Health Therapy/Rehabilitation

Schedule: 08:00 - 17:00

Contract Length: 13 weeks

Candidate Type: Traveler

Requirements: Must have home health experience; OASIS training preferred Certs required: BLS, state licensed Must have valid drivers license and car to make home visits. Must be COVID vaccinated or with exemption form on file. Must have flu shot or declination and in both cases mandatory mask wearing at all times while at work.

Responsibilities:

• Provide skilled home health therapy services to patients in their homes.
• Assess patient needs and develop individualized care plans.
• Document patient progress and maintain accurate records using BayCare Forms, BayCare Scheduling, and Ace Timesheet documentation system.
• Educate patients and families on health management, medications, and safety measures.
• Collaborate with the healthcare team to coordinate patient care and support recovery goals.
