Job Details:
Job Title: Case Manager Registered Nurse
Location: Williamsburg, VA
Setting: Home Health
Pay Range: 2301.00 - 2416.05
Schedule: 10:00 - 18:00
Contract Length: 13 weeks
Candidate Type: Traveler
Requirements: Home hospice case management experience; BLS; EPIC
Responsibilities:
- Manage home hospice cases for a census of approximately 35-45 patients.
- Provide client-centered care and coordinate services for hospice patients in the home setting.
- Document care plans and patient progress using EPIC.
- Communicate effectively with patients, families, and interdisciplinary teams.
- Adhere to all safety and compliance protocols related to hospice care.
