Job Details:

Job Title: CWHH-Physical Therapist

Location: Griffin, GA

Setting: Home Health

Pay Range: 2495.60 - 2620.38

Schedule: Discuss with recruiter.

Contract Length: 13 weeks.

Candidate Type: Traveler

Requirements: Current State License as a Physical Therapist; Proof of current CPR certification; Current driver's license. Must have reliable transportation and insurance; 1-2 years experience as a Physical Therapist in an acute care or rehabilitation setting.

Responsibilities:

• Perform clinical assessments to identify patient needs and appropriateness of care.
• Document, review, and complete a Plan of Care (POC), progress notes, and clinical records, revising based on ongoing assessment data.
• Plan and provide treatment for neuromuscular, neurological, and orthopedic disorders based on referrals and assessments.
• Assist and instruct patients and families in transfer techniques and the use of equipment such as walkers, wheelchairs, canes, braces, and prosthetic devices.
• Educate patients and families regarding therapeutic procedures tailored to age, condition, culture, and readiness.
• Oversee other team members in patient care activities appropriate to patient needs.
• Apply infection control and universal precautions to prevent contamination and transmission of disease.
• Accept clinical assignments aligned with education and competence.
• Perform other related duties as assigned.
