Requirements: 1 year of Home Health and OASIS experience, HomeCare HomeBase EMR experience
Responsibilities:
- Provide skilled nursing care in a home health setting.
- Follow physician orders and develop patient care plans.
- Educate patients and families on health management.
- Monitor and document patient progress.
- Collaborate with healthcare team members to ensure quality care.
